It’s here to help you understand what a U.S. surrogate may typically receive and to give you space to begin shaping your own personalized package.

🌿 Base Compensation: $40,000 – $120,000 USD
Your base compensation reflects the respect and appreciation for your incredible role in bringing life into the world.
The first installment of base compensation will be disbursed on the first (1st) day of the calendar month after pregnancy is medically confirmed, provided the Gestational Carrier remains pregnant with a viable fetus at that time.
Base compensation is divided into 10 monthly installments, and any remaining balance will be issued within five (5) days after delivery.

If pregnancy is confirmed through HCG testing but a fetal heartbeat is never detected, or if a miscarriage occurs prior to the first scheduled payment, the Gestational Carrier will not receive the first installment but instead will be provided a $500 Miscarriage Fee.

If delivery occurs at or after 36 weeks, full compensation will be paid even in the unfortunate event of stillbirth.

If delivery occurs before 36 weeks and the baby does not survive, no remaining balance will be due.
-
If delivery occurs before 36 weeks and the baby survives, full compensation will be released upon the newborn’s discharge from the hospital.
🌿 Monthly Allowance - 200 USD to 600 USD
We understand that small, everyday expenses add up. To ensure you are never burdened by them, we provide a monthly allowance beginning after legal clearance and continuing through the month of delivery.
This allowance covers:

Local transportation (within 30 miles round trip)

Phone, internet, fax, or postage

Vitamins, supplements, or small health items

Childcare during brief appointments

Snacks or meals related to travel or medical visits

🌿 Maternity Clothing Allowance
Your comfort and dignity matter deeply. As your body changes to nurture new life, we want you to feel supported, confident, and comfortable at every stage.

This allowance is intended to help you purchase maternity clothing that makes you feel good — because you deserve comfort that reflects your compassion and the extraordinary love you’re giving to another family.
🌿 Medication, Cycle & Transfer Support
Every medical step you take brings a family’s dream one step closer to reality. These compensations are designed to honor your time, effort, and cooperation during the medical process.

Medication Start Fee – $500:
This fee is provided at the start of hormone injections (when applicable) to help offset related expenses. -
Embryo Transfer Fee – $500:
Paid for each completed embryo transfer, this inclusive fee covers up to three (3) consecutive days of lost wages, childcare, and rest time. -
Canceled Cycle Fee – $500:
If a cycle is canceled through no fault of your own after medication begins, you will receive this fee in recognition of your time and effort. -
Mock Cycle Fee – $500:
Provided for completing a mock (trial) cycle when requested by the fertility clinic or physician.

Every item in this section was created with your comfort, safety, and dignity in mind. From travel and childcare to clothing and medical support, these provisions exist to ensure that your journey as a surrogate is one of respect, balance, and genuine care — both for you and your family.
🌿 Invasive Procedure Compensation
In the event you must undergo an invasive medical procedure because of the surrogacy arrangement, you are entitled to the following compensations (inclusive of childcare, housekeeping, and recovery support):

Amniocentesis / CVS $500 per needle
Cervical Cerclage $500
Uterine Biopsy $500
Loss of One Fallopian Tube or One Ovary $1,000
Loss of Both Fallopian Tubes or Ovaries $3,000
Termination (before 13 weeks) $1,000
Termination (after 13 weeks) $3,000
Selective Reduction $1,000 per needle
Ectopic Pregnancy Surgery $3,000
Why this matters: Your health and well-being matter deeply. We make sure you’re supported financially and emotionally should a medical necessity occur.
🌿 Multiple Pregnancy Bonus
Carrying more than one baby takes additional physical and emotional strength, and your effort deserves to be recognized.
If you are carrying multiples, you will receive an additional $10,000 per extra fetus, paid as a bonus throughout your journey.
This additional compensation reflects our deep appreciation for the extra care, time, and commitment required for a multiple pregnancy.
🌿 Breast Milk Compensation Details
If you and the Intended Parents agree and you choose to provide breast milk, you will be compensated:

USD 300 per week (or per this agreement) for each week you provide milk.

We reimburse all pumping supplies, shipping costs, and necessary equipment with documented receipts.
Why this matters: Breastfeeding is a generous and physically demanding choice. We value your contribution.
🌿 C-Section, Miscarriage & Organ Loss Support

C-Section Compensation: USD 3,000 if a medically indicated C-section occurs.

Miscarriage Compensation: USD 1,000 in the event of a miscarriage prior to the agreed milestone.

Loss of Uterus Support: USD 5,000 if loss of uterus with cervix; USD 3,000 if loss of uterus alone.

🌿 Counseling & Wellness Support Details
You are eligible for up to 8 individual counseling sessions, each reimbursed at USD 100, to support your emotional health, well-being, and reflect the remarkable journey you are helping others experience.
Why this matters: You are not just providing physical service—you are emotionally engaged in a profound way. We honor that and invest in your mental health.
🌿 Initial Escrow Deposit
To ensure complete financial transparency and protection, 110% of the total benefit package will be securely deposited into escrow before your journey begins.
This guarantees that every payment outlined in your contract is fully funded and available throughout your surrogacy experience.
🌿 Accidental Death and Disability Insurance
Your safety and your family’s security mean everything to us.
A comprehensive Accidental Death and Disability Insurance policy will be in place from the time of legal clearance through embryo transfer, providing:

These provisions are designed to safeguard you and your family, offer practical assistance during special circumstances, and ensure that every aspect of your courageous and compassionate contribution is respected, protected, and honored.
🌿 Loss of Wages During Pregnancy
If you are unable to work due to medical appointments, bed rest, or pregnancy-related conditions directly connected to your surrogacy journey, your actual net income loss (after deductions) will be reimbursed.
This benefit is provided to ensure your financial stability throughout the pregnancy and to recognize that your time, comfort, and health are essential.
Reimbursement is based on the actual verified loss of wages, supported by recent pay stubs or tax records.
A cap may apply depending on the specific terms agreed upon with your Intended Parents and outlined in your legal contract.
This coverage ensures that you remain financially secure even when medical care or rest takes priority over work.
🌿 Maternity Recovery Benefit (Post-Delivery Compensation)
Your recovery and well-being after delivery are deeply important to us.
To ensure you have the time and financial support to rest, heal, and transition comfortably after birth, Smart Surrogacy provides Maternity Recovery Compensation to cover lost wages during your recovery period.
Your actual net income (after deductions) will be reimbursed, verified through documentation such as pay stubs or tax records.
Compensation limits:

Up to 4 weeks following a vaginal delivery

Up to 8 weeks following a C-section
-
Up to 12 weeks if additional medical complications arise and a doctor’s certification is provided
This benefit is designed to reflect the same care and respect given to traditional maternity leave — because your recovery matters just as much as the beautiful life you’ve helped bring into the world.
🌿 Loss of Wages – Spouse/Partner
If your spouse or partner must miss work due to the surrogacy process, their actual lost wages will also be reimbursed (after deductions), according to the following limits:

Court appearance: up to 1 day

Travel for embryo transfer: up to 3 days

Invasive procedure: up to 1 day

Hysterectomy: up to 2 days

Vaginal delivery: up to 3 days

C-section: up to 5 days
Reimbursement is based on documented, actual wage loss (for example, pay stubs, employer letter, or tax records) and will not exceed the income that was truly lost.
These benefits are intended to ensure that your partner’s presence and support during key moments of your journey are never a financial burden.
🌿 Local Travel
Your time and commitment are deeply appreciated. To ensure that no surrogate bears any out-of-pocket costs for necessary travel, Smart Surrogacy will reimburse you for local transportation as follows:

If the distance traveled is greater than 50 miles round trip, reimbursement will be calculated based on the current IRS-approved mileage rate at the time of travel.

Mileage reimbursement applies to all surrogacy-related appointments, including monitoring visits, legal appointments, and any medical procedures.
This provision ensures that you are fully supported for every journey you make in helping a family grow.
🌿 Lodging and Meals
When travel requires you to stay overnight, Smart Surrogacy will provide reimbursement for meals and accommodations to help make your travel as comfortable as possible:

Overnight trips: up to $50 per day per adult
-
Non-overnight trips: up to $35 per day per adult
These reimbursements cover reasonable lodging and meal expenses so that you can travel for appointments or procedures with ease and peace of mind.
🌿 Transportation During Travel (Non-Local Travel)
For medical appointments or procedures requiring regional or out-of-town travel, Smart Surrogacy will reimburse transportation costs such as:

Rental car, Uber, Lyft, taxi, or comparable services
-
Up to $150 per day, based on submitted receipts
This ensures that your travel outside your local area remains comfortable, convenient, and fully covered — allowing you to focus entirely on your health and the success of your surrogacy journey.
🌿 Childcare Expenses During Overnight Travel
If childcare is needed during overnight travel and your children are not being cared for by you or your spouse/partner (if any), actual childcare expenses will be reimbursed based on receipts or invoices, up to a maximum of USD 100 per day.
🌿 Housekeeping and Childcare Support During a Normal Pregnancy
Your health, balance, and peace of mind are always important to us.
During a typical pregnancy, if you occasionally need extra help caring for your home or children — especially when attending appointments or resting after procedures — Smart Surrogacy will provide limited reimbursement to support your comfort and stability.
If your children are not being cared for by you or your spouse/partner, actual childcare expenses will be reimbursed upon submission of receipts or invoices, within the following limits:

From pregnancy confirmation through week 29: up to $100 per month

From week 30 through delivery and up to two (2) weeks after delivery: up to $100 per week
This support is designed to ease minor household or childcare responsibilities during your pregnancy, allowing you to focus on self-care, rest, and the healthy growth of the baby you are carrying — because your comfort and dignity deserve our highest care and respect.
Note: During any period when you are receiving “Bed Rest and Housekeeping Support (Medical Necessity),” this normal pregnancy childcare allowance is paused, and only the higher bed rest benefits apply for those days.
🌿 Bed Rest and Housekeeping Support (Medical Necessity)
If at any point your doctor recommends bed rest, restricted activity, or limited mobility, additional support will be provided to protect your health and well-being.
With a physician’s note confirming medical necessity, you may receive reimbursement for:

Up to $200 per week for housekeeping assistance; and
-
Up to $50 per day for childcare (up to 7 days per week)
for a maximum of up to twelve (8) weeks, based on actual expenses and supported by receipts or invoices.
This enhanced level of support is meant to ensure your household continues to function smoothly while you focus fully on your recovery and the baby’s safety.
